Info A3 Sheet on the bottom of Pinball Cabs

Any idea how or where i could get or recreate the A3 paper sheet with info that is placed in the bottom or pin cabs.Mine is really F"d up and as i,m doing a full restore would really like to get a new one printed out then laminated to finish her off,

Which game is it for ?

The proper size should be ledger.. i think its 11" X 17" ... its just under A3..

I recently printed out a copy for my STTNG and went and had it laminated at officeworks and it cost me a measly $3.75 to laminate it !
