I have seen some really weird shit happen on Bally/WMS 90s games when the game software goes corrupt.  On my fishtales recently the fishing reel would not rotate.  Ok in all test modes and all optos ok in test modes.  Replaced software and fixed..Bizzare...put old software in to confirm and yes fault back on...corrupt software a possibility after you have checked all the normal issues
